Applicants can apply in person, without a lawyer or representative, through their local Social Security Administration offices, through the mail, or on-line at the Social Security Administration Web Site (link provided from home page).
To successfully apply for Social Security Disability benefits, the applicant must compile detailed and accurate information about their injury or illness; medical records (including the names, addresses, phone numbers and dates of service for all doctors, therapists, hospitals, clinics which provided you care); work history (for the past 15 years); and life style (education, family, hobbies), in the manner in which Social Security Administration requires, during a time in their lives when they must also deal with high levels of pain and stress.
This process is difficult and disappointing as more than 60% (and some have estimated as high as 79%) of the initial applications for disability are denied. However, approximately 60% -70% of these denials are eventually successful in having their claims accepted after either reconsideration or via a hearing before an Administrative Law Judge.
